Requirements to fill the form:	§    One passport size photo scanned file (big size)   §                Signature scanned file   §  Identity Proof for later verification. (Voting card,       Aadhar card, PAN card etc.)   §  Mark list of the degree as a proof.
Fees for General and OBC: Fees for all others:	400/- 50/-
